ENROLLMENT Forms

To begin the enrollment application and registration process, please :

  1. Complete the Enrollment Application, Code of Conduct and Customer Agreement forms.
  2. Submit all completed forms as attachments in an email to help@friendsshineonline.com.
  3. After we review your application, we will contact you to set up a Google Meeting so that we can meet each other and answer any questions you may have.

All necessary forms are available below.

When you open a form, please note they are in an interactive pdf format which allows you to complete the form digitally, save the completed form to your computer, and email it as an attachment to help@friendsshineonline.com. You must have a program like Adobe Reader installed to view the forms.

We do not collect, store or share any personal information on the Friends Shine Online Website.
